Thank you for being part of our network of eye care professionals. For the latest information and resources, we encourage you to join our provider network and register for our Eye Health Manager, where you can check member eligibility, vision benefits, plan specifics, and clinical policies, in addition to submitting authorizations and claims.
You can also verify member eligibility through our automated IVR system, available 24 hours a day, seven days a week. To speak with a Provider Services representative, call our Provider Services line Monday through Friday, 8 a.m. to 8 p.m. local time.
Ambetter from Sunshine Health (Marketplace)
Ambetter Health: Health Insurance Marketplace plans for individuals and families who may not qualify for Medicaid or other government coverage.
Ambetter Health Solutions: Health Insurance Marketplace plans for those who receive a defined contribution from their employer to purchase individual health insurance, such as through Individual Coverage Health Reimbursement Arrangements (ICHRAs).

Sunshine Health Managed Medical Assistance is a managed care plan providing Medicaid and enhanced benefits to Florida residents of all ages who qualify for Medicaid.

Sunshine Health is a Managed Care Organization (MCO) that has contracted with the State of Florida to provide covered health benefits for foster care beneficiaries under the Pathway to Shine program.
